How to Apply for/or Renew Your Colorado Medical Marijuana Registry Card 

1. See your Healthcare Provider

Your healthcare provider will write a provider certification for you and submit it electronically. Finish the rest of your application within 6 months of the date your provider certification was issued.

  • Get a printed copy of the provider certification for your records.

  • Ask your healthcare provider to check that all information on your certification is correct. 

  • Submit a new provider certification each time you apply.

More information for Minor applicants
  • All minor patients must see two providers and submit two physician certifications.

  • If the minor patient has a  disabling medical condition, which includes Post Traumatic Stress Disorder and Autism Spectrum Disorders, and any condition for which a physician could prescribe an opioid, and the recommending physician is not the patient’s primary care physician, the physician should review the records of a diagnosing physician or a licensed mental health provider acting within his or her scope of practice. For more information please refer to C.R.S. 25-1.5-106 and House Bill 19-1028.

More information for applicants aged 18-20
  • Applicants in this age group that did not have a registry card before age 18 and applied for the first time on or after Jan. 1, 2022 will need provider certifications from two different providers at different medical practices each year when they apply.

  • If the applicant in this age group is homebound, the providers do not need to be from different medical practices.  If you are an applicant in this group, make sure both of your providers enter their certifications into the Medical Marijuana Registry system. Check to make sure all of your identifying information is correct and matches on both documents. Then, get copies of the certifications from your providers.

 

2. Complete your application online

 Check the list of required documentation 

Make sure you have all the required information. Applications must be submitted by the patient or the patient's legal representative.

Enter your information
  • Renewal applicants log into your account and click on your name under "Tracking Inbox: Patient Registration." Then scroll down and click "Save and Start Application." 

  • First-time applicants create an account and click “New” under "Tracking Inbox: Patient Registration." 

When you are ready to move to the next step, click "Save & Submit Patient Application."

Add your Provider Certification
  1. Click the “Provider certification” tab

  2. Click “New”

  3. Choose your provider certification from the drop-down menu

  4. Click “Save”

Applicants age 18-20

Applicants in this age group that did not have a registry card before age 18 and applied for the first time on or after Jan. 1, 2022 are required to submit two provider certifications. Currently, the system will only allow you to add one certification to your online registration. Registry staff will add the other one for you after you submit your application. Please make sure all of your identifying information is correct and matches on both provider certifications. 

Choose your Cultivation Information
  1. Click the “Cultivation information” tab

  2. Click “New”

  3. Select your cultivation option and complete the required fields

  4. Click “Save”

If you are applying with a caregiver, make sure that you have your caregiver's registration ID number. Your caregiver will need to accept a caregiver agreement in order to be listed on your card. 

Pay

The application processing is fee is $29.50. It is non-refundable and is required each time you submit an application.

To pay with a credit card, debit card, or electronic check 

  1. Click the "Payment" tab

  2. Click "New"

  3. Choose "Click here to pay." This will open a new window where you will enter your payment information. 

To pay with a fee waiver

  • Submit a copy of your certified Colorado tax return for the current tax year. You can get this from the Colorado Department of Revenue.

  • You may qualify for a fee waiver if your household income is 185% of the Federal Poverty Level or less. Review annual adjusted household incomes here: https://aspe.hhs.gov/poverty-guidelines

 

3. CDPHE will review your application 

Applications submitted online with all correct information are approved in 1-3 business days.

Applications take longer to process if they are submitted with incorrect information or expired documents.

Once your application has been processed you will get an email to the address you used to register. 

 

4. Print or Display your card

Once approved, you can access your card through your online account.

  • Click on your name under "Tracking Inbox: Patient Registration."

  • Click the “print card” button to print your card or display it from a mobile device after you have been approved.

  • If you applied online you will not receive a card in the mail. 

 

The issue and expiration dates on your card will be based on the day your application was approved. 

The Marijuana Enforcement Division (MED) at the Colorado Department of Revenue has new rules, effective Dec. 1 2022, that clarify what information is required when designating a primary medical marijuana center. Please contact the Marijuana Enforcement Division if you have questions about the changes.

If you need a copy of your provider certification, follow these steps:

  1. Log in to your patient account by entering your username and password.

  2. Under "Tracking Inbox: Patient Registration", click on your name next to “Active”

  3. Select the "Provider Certification" tab on the top row of tabs

  4. Click on your most recently issued provider certification

  5. You can then select “Print Friendly Format” to access the certification and print it if you need to.
