How to Apply for Your New Mexico Medical Cannabis Program Card (NM-MCP)
1. Gather Required Documentation
Before applying for a medical cannabis program card, you will need to gather the following documents:

Valid New Mexico Identification: A New Mexico driver’s license or state ID card will suffice

Medical records that support the qualifying condition: You will need to provide medical records from a healthcare provider that support your qualifying condition. This includes a diagnosis and documentation of ongoing treatment.

For those under the age of 18, a clear copy of the birth certificate and a completed “Caregiver Application” and the documents needed for that application

"Primary caregiver” means a resident of New Mexico who is at least 18 years of age and who has been designated by the qualified patient or their representative and the patient’s practitioner as being necessary to take responsibility for managing the well-being of a qualified patient with respect to the medical use of cannabis pursuant to the provisions of the Lynn and Erin Compassionate Use Act, Section 26-2B-1 et seq., NMSA 1978.

Minors (children under age 18) are required to have a Primary Caregiver.

2. Schedule an Appointment with a Qualified Healthcare Provider

If you are interested in applying to the New Mexico Department of Health, Medical Cannabis Program, you must first visit your medical provider. Your Medical Provider will conduct a medical assessment to identify the qualifying condition that supports your use of medical cannabis.

Based on the medical assessment, your medical provider will start the Patient application.

When working with an authorized medical provider, you can complete and submit your applications electronically. Applications submitted through the Online Patient Portal are typically processed within 5 business days; you can access your electronic card upon approval (see instructions).
3. Submit Application to the New Mexico Department of Health

The Online Patient Portal allows qualified patients to have 24-hour access to check the status of their application, check their available units, review their sales history, and access their electronic card

Applications submitted through the Online Patient Portal are typically processed within 5 business days; you may access your electronic card upon approval. Go to https://mcp-patient-tracking.nmhealth.org

You will need to provide personal information, a copy of your valid New Mexico identification, and the medical cannabis program certification form from your healthcare provider.

You will also need to pay a fee when submitting your application.
4. Wait for Approval

Once your application has been submitted, it will be reviewed by the New Mexico Department of Health.

The review process can take up to 30 days.

If your application is approved, you will receive your medical cannabis program card in the mail.

If your application is denied, you will be notified and provided with an explanation of the decision.
